could also be a failure in fuel pump/flow and landing gear...
whatever it is ultimately it comes back to crew error(s)...

I think it was a lack of air in the tyres. Did you see that some dumbarse has blocked the radiator with panels? I mean how is the engine supposed to cool it's self?

Maybe you should be there, so if the plane is not going fast enough, they could let you out and you could push it. That would help.
